Privacy Policy

Soul Nutrition WA may ask for personal information in the course of providing a health service which is considered health information under the Privacy Act. Health information is ‘sensitive information’. Soul Nutrition WA handles your private health information with the utmost care. Your information is stored in a protected digital record which is required for the documentation of the consultation in order to provide the most effective service possible.

Types of information recorded

• Recording what you say, or recording their opinion about what you have said

•Personal information such as name, address, date of birth, Medicare number and medical history

• Keeping a specialist report from another health professional provided by a you for inclusion in the patient’s medical record

• Record of clinical or biochemical information

• Keeping emails or other correspondence containing personal information about a patient

Disclosure

With your consent, some of your health information may be disclosed to other relevant organisations involved in the care of yourself as the patient. This includes:

•Sharing health information with another health service provider or individual relevant in your care

•Discussion of your name and identifying information with reception staff in another relevant health care or medical centre

Under no circumstances is your health information shared to overseas recipients.

Any breaches to the Privacy Act can be made to www.oaic.gov.au/privacy/privacy-complaints where it will be handled by the Office of the Australian Information Commissioner (OAIC).
